Pharmaceutical Care is constantly moving.
Optimizing medicine use and improving health outcomes is understood as the pharmacist’s contribution to the care of individuals. With the rapid development of new drugs, new ways of their application, and enlargement of treatment options and technologies, pharmacists in community and hospital pharmacies also have a growing and important role within multidisciplinary healthcare teams. In this session, we will explore innovations in pharmaceutical patient care to reduce potentially preventable medication-related harm. Our speakers will show the need of pharmaceutical care in preventing drug-related hospital admissions, reduce falls due to adverse drug reactions and better communicate between primary and secondary care on the duration of antithrombotic treatment.
